HopeHealth is seeking an experienced Registered Nurse for its affiliate HopeHealth Hospice & Palliative Care. This position will be based in Providence, RI.

The Referral Center Registered Nurse (RN) acts as a customer service liaison between referral sources, care providers, professional staff, patients and families, insurers, physicians and members of the IDG to ensure the smooth and timely admission of patients. S/he ensures timely response to all referrals and oversees coordination of referral center staff, including assigning daily tasks, to respond to and follow-up on all referrals in a timely manner. S/he, along with hospice "doctor of the day" determines appropriateness of patients for admission to service, including level of care, including consulting with other referral center staff to do the same.

S/he performs these duties as prescribed by the physician and in compliance with regulations and accepted standards of practice as established by the State Board of Registered Nursing, accrediting bodies, and the policies of Hope Hospice & Palliative Care Rhode Island.

 

Duties / Responsibilities:

  • Oversees daily operation of the referral center to ensure timely response to all referrals and mentors new staff in their orientation to the role.
  • In conjunction with the Patient Care Manager, identifies opportunities to develop new or improve current processes within the referral center to improve overall operations.
  • Demonstrates commitment to patient care utilizing outstanding communication skills throughout the entire referral and admissions process.
  • Schedules visits for all inquiry and referral callers within required timeframe. Arranges visits for liaisons and admissions nurses. Provides follow up on pending patients and non-admissions. Communicates prospective patient updates to appropriate referral sources.
  • Follows admissions management policies and procedures. Obtains information and accurately completes all documentation, forms, and logs for referral calls and admissions processing. Dispatches information and coordinates visit schedules for visiting pre-admission staff.
  • Communicates with referral sources, DME, pharmacy, and other vendors to process all certifications, orders, and required admission information. Communicates with patient accounts to verify insurance and authorize care. Meets 24/7 call and visiting standards.
  • Facilitates equipment and other admission orders as needed. Enters prospective and admitted patient demographic information and status updates into patient database as needed.
  • Facilitates processing of admission documents from referral calls through to transfer to the appropriate care team in a timely manner.
  • Helps resolve referral and admissions scheduling issues and customer complaints through direct communication with the referral sources, admissions team, Help Center & Admissions Manager and other managers.
  • Makes follow up call for referral inquiries with required time.
  • Provides clinical support for Referral Center and Admission nurses. Assumes role of Admission Nurse as needed.
  • Provides clinical follow up with referral sources on upcoming discharges from hospitals and LTC facilities to hospice.
  • Participates in weekend rotation with other Referral Center RNs.

 

Education / Experience:

  • Associates degree in Nursing required, Bachelor of Science Degree in Nursing preferred.
  • One to three years of related experience or training; or related equivalent combination of education and experience.
  • Home care and/or hospice experience preferred.
  • Rhode Island and Massachusetts license in Nursing.  
  • Ability to read, analyze and interpret professional journals, basic financial reports, medical records and legal documentation skills.  
  • Ability to work with mathematical concepts and to calculate medication dosages.
