HopeHealth is seeking an experienced Care Navigator.  The Care Navigator will actively seek out opportunities to develop and deepen relationships in long term care facilities and assisted living communities within Rhode Island. The Care Navigator is responsible for achieving growth goals in an assigned territory with a customer centric approach.  The Care Navigator acts as the primary representative for HopeHealth, facilitating the navigation of patients into the appropriate hospice, palliative care, or home care services.

 

Duties / Responsibilities:

  • Achieves territory goals and actively participates in the implementation of marketing plan.
  • Works closely with assigned facilities to foster deep and trusting relationships to help achieve optimal patient care leveraging HopeHealth's continuum of services.
  • Actively works with the referral center, other care navigators, home care, hospice and palliative care teams in ensuring smooth care transitions with the goal of achieving the highest patient care and satisfaction.
  • Establishes and maintains very strong working relationships with referral sources including healthcare professionals, facility staff and the community at large.
  • Provides education to patients, caregivers, families.  Serves as the subject matter expert in all HopeHealth services
  • Actively participates in community and industry events in support of relationship development, brand recognition and census growth.
  • Leverages HopeHealth educational resources and programs in cultivating and growing relationships across key audiences.
  • Aware of HopeHealth's overall growth strategy and supportive of new initiatives as required to support growth.

 

Education / Experience:

  • Bachelor's degree with 3+ years of experience in healthcare.  Business Development experience preferred.
  • Clinical background (nursing, social work, etc.) preferred but not required.
  • Excellent interpersonal skills to related effectively to all customers including patients, families, providers, administrative leadership, staff and the community.
  • Strong written and verbal communication skills which will provide a positive atmosphere for patient, family and primary caregiver(s).
  • Compassionate, empathetic and self-disciplined individual committed to the strong values of the company's mission.
  • Valid driver's license, automobile insurance and own transportation.
  • Prior hospice experience preferred but not required.
