HopeHealth is seeking an experienced Medical Social Worker for its affiliate Hope Hospice & Palliative Care Rhode Island.

This is a per diem position.


Duties / Responsibilities:

  • Implements standards of care for Medical Social Work Services and ongoing patient care services.  Medical Social Services are provided by a qualified social worker (MSW) in accordance with a plan of care.
  • Assessing the psychosocial/emotional status of patient/family/caregivers related to patient's terminal illness.
  • Assisting the physician and other IDG members in recognizing and understanding the social/emotional stress and/or disorder that exacerbates the symptoms related to terminal illness.
  • Assessing the bereavement risk and need for counseling related to grief issues.
  • Assessing environmental safety, suicide potential/risk, and abuse or neglect risk.
  • Assessing community resource, legal/financial and funeral planning needs and ability to access services.
  • Assessing caregiver's ability to function adequately.
  • Assessing special needs related to cultural diversity including communication, space, role of family members and special traditions.
  • Evaluating for long range planning and assessing ability to accept change in level of care.
  • Evaluating patient/family/caregiver response to psychosocial interventions.
  • Identifying family dynamics and communication patterns and obstacles to learning or ability to participate in the care of the patient.
  • Identifying support systems available to reduce stress and facilitate coping strategies.
  • Identifying obstacles to compliance and assisting in understanding goals and interventions.


Education / Experience:

  • Master's Degree in Social Work from an accredited school and a will be a state registered independent social worker (LCSW or LICSW in RI) preferred
  • LCSW accepted while working toward LICSW
  • Two to five years of home care and/or hospice experience preferred
  • Gerontology experience preferred
