HopeHealth is seeking an Hospital Liaison.  HopeHealth's headquarters are based in Providence, Rhode Island.  Its affiliate members are Hope Hospice & Palliative Care Rhode Island, Hope Palliative Care Center, HopeHealth Massachusetts and Visiting Nurse of HopeHealth.

The Hospital Liaison acts as the primary representative of HopeHealth in designated facilities providing skilled assessment of patients for eligibility for the hospice benefit in collaboration with the hospice medical director and facility staff. The liaison ensures compliance with all Medicare/Medicare and State for hospice admission.

The expected geography of this position is Miriam and Sturdy Hospitals.


Duties / Responsibilities:

  • Serves as the primary representative of the HopeHealth hospice admission team in designated facilities. Creates a collaborative and consultative environment with hospital staff working directly with case management staff to perform care management of patients referred for hospice services.
  • Provides education on the hospice benefit and HopeHealth services to patients, care givers, families and providers. Provides professional education opportunities to facility staff to teach about the hospice benefit and supports organizational growth in designated facilities partnering with HopeHealth business development, Palliative Care Services and home health as needed.
  • Provide initial assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living to determine eligibility for the hospice benefit. Presents hospice in a manner to eligible patients that highlights the benefits of hospice in caring for individuals with a terminal diagnosis.
  • Implement the individualized plan of care for admission including documenting the Local Coverage Determination (LCD).
  • Assists in the coordination of discharge planning, as needed, with facility case management staff for patients enrolling in HopeHealth services.
  • Uses interdisciplinary approach in developing, plan of care, in accordance with the organization's policies. Initiates referrals to other disciplines and refers to community resources only when necessary to meet an immediate need.
  • Documents in EMR and meets department expectations for completing in a timely manner in order to provide optimal response to all patients.
  • Ensures that patient information regarding payer source is current and correct. Works with the organization to obtain necessary insurance information and coverage.
  • Provides other duties as assigned, including but not limited to, field visit coverage as needed.



Education / Experience:

  • RN required BSN preferred
  • One to three years of related experience or training; or related equivalent combination of education and experience
  • Home care and/or hospice experience preferred
  • Rhode Island or Massachusetts license in Nursing required depending on location; Dual license in RI and MA strongly preferred
  • Ability to read, analyze and interpret professional journals, basic financial reports, medical records and legal documentation skills
  • Ability to work with mathematical concepts and to calculate medication dosages
