HopeHealth is seeking an experienced Quality Data Associate.  HopeHealth's headquarters are based in Providence, Rhode Island.  Its affiliate members are Hope Hospice & Palliative Care Rhode Island, Hope Palliative Care Center, HopeHealth Massachusetts and Visiting Nurse of HopeHealth.

Under the direction of the Director of Quality and Compliance supports organization-wide compliance, quality and process improvement projects and initiatives. Access and utilize data from multiple sources, (including electronic medical records, SHP, audit tools, incident reporting, compliance reports), to create and maintain reports, dashboards and presentations that enhance quality improvement metrics and compliance initiatives through all departments in the organization.

Responsibilities include:

  • Organizes multiple data inputs and creates comprehensive, targeted reports to support decision making at all levels of the organization; including senior leadership, managers and clinical teams. Interface with departments to collect and distribute data used to analyze and benchmark progress.
  • Maintain systems for reporting and analysis of complaints and incident frequency and mitigation. Supports the Quality & Compliance department in creation of annual compliance and quality plans, responses to external accrediting and auditing agencies and payers. Participates in annual policy updates, special projects with affiliated agencies and quality-based research initiatives.
  • Maintains and streamlines manager's system for reporting and response to fiscal intermediary and other external auditing agencies. Supports organization-wide survey and accreditation preparation and response initiatives. Collects monthly data and submits reports for ongoing benchmarking projects.
  • Collect information from various internal and external sources and information systems, compile the data, and summarize findings for monthly and ad-hoc information requests while meeting designated deadlines with high level of accuracy.
  • Creates and updates systems for quarterly distribution of data to teams including CAHPS/Joint Commission survey data, quality metrics, medical record audits, performance improvement projects and compliance data and reports.
  • Creatively responds to requests from managers for data to support operational decision making.
  • Support development of the annual quality and compliance plans and associated work plans. Prepare reports and analyses of trends with particular emphasis on presentation of findings and outcomes.
  • Prepare monthly reports to meet departmental and customer needs from various systems and reports. Interfaces with clinical management team to promote timely completion of documentation associated with these processes.
  • Manages all data associated with incident and complaint response and follow up. Creates and formats reports for presentations at Board and leadership meetings.
  • Organizes annual policy reviews across all departments and schedules quarterly policy and procedure committee meetings.
  • Completes scheduling and agenda preparation for quality & compliance integration team meetings and other committees as requested.
  • Work to support Quality& Compliance Department with the goal of meeting and exceeding department and customer needs. Interface with various departments and functions outside of the department.
  • Consistently apply the HopeHealth values in all aspects of work, complying with established HopeHealth mission, policies, procedures and objectives in quality assurance, safety, environment, infection control and HIPAA compliance.

 Education / Experience:

  • Bachelor's Degree required, preferably in Math, Social Science, Healthcare, Science or related area.
  • One to three years' experience or equivalent combined work/educational experience with proficiency in data extraction, management, and presentation.
  • Experience in healthcare setting such as hospitals, hospice, homecare, or physician practice desired.
  • Demonstrated proficiency in utilizing Excel, Word, Windows, Outlook, Explorer and Power Point applications.
  • Knowledge of Excel functions, such as pivot tables and graphical presentation a plus Microsoft Access experience a plus.
  • Knowledge of flow charting and database applications a plus.
